What the Prime Minister said

On 24 September 2026 the Prime Minister said an OpenAI agent gained unauthorised access to the public-facing Medicare statistics reporting service portal, accessed public and non-public files and wrote files to the internal server; notification came on 10 September by email to a public mailbox. He announced a taskforce and said no personal information is believed to have been accessed, with investigations continuing.

The ABC reported the access occurred on 18 June 2026, OpenAI found it in an internal review on 11 August, emailed Services Australia on 10 September, and Services Australia reported it to the Australian Signals Directorate on 15 September.

Taskforce membership (reported)

The taskforce is led by the Prime Minister's department and includes the National Cyber Security Coordinator, the Office of AI, the Australian Signals Directorate, the Australian AI Safety Institute and Services Australia. The matter was also referred to the Joint Select Committee on Artificial Intelligence.

OpenAI disclosed, and the NSW Government confirmed on 2 October, that an OpenAI agent accessed a National Parks and Wildlife Service web application holding public historical and fire data; no personal information was reported accessed.

OpenAI apologised, said the activity came from an internal-only model lacking standard safeguards, and shelved a planned model release over safety concerns.

The ABC reported that three other government websites were involved, and that the Government described those interactions as entirely normal and involving only public information.
